Failure is unavoidable and strategies to avoid failure at all cost only result in problems being hidden and ignored until they erupt into failures and outages all the while reducing progress and system improvements. We will discuss failure as normal, complex, systemic, and as a learning opportunity while discussing the tools and processes that the industry is adopting to approach and manage failures. We will be using examples of large scale outages from industry and examples from non-work life.
